What is Breast Augmentation?

Breast augmentation surgery is a procedure to increase or restore the size of the breasts using silicone gel implants or saline implants. It is one of the most popular plastic surgery procedures performed to date and has a long and successful track record in helping women restore balance to their figures. Many women experience changes in their breasts due to weight loss, pregnancy, or aging, and a breast augmentation can restore a more youthful and symmetrical breast appearance for a boost in confidence.

There are numerous breast implant choices, including shaped implants (sometimes called “Gummy Bear” implants); gel-filled (once called “silicone implants”); and saline implants. Choices in Breast Implant Incision Locations

There are three primary choices for the incision used in breast augmentation procedures. An incision can be placed:

  • Beneath the breast in the crease where the breast meets the chest wall (called the inframammary fold);
  • In the underarm area, or
  • Around the areola.

Although each woman’s anatomy is different from another’s, the choice of incision is most influenced by the size and type of implant as well as the incision that appeals to you the most. Dr. Berman will discuss your incision options during your consultation appointment.

A Caveat About Axillary Breast Implant Placement

Depending upon your age, you may wish to avoid an underarm (axillary) implant placement. While today’s breast implants are extremely sturdy, it’s logical that most women will wish to replace their implants after 10-20 years or if they experience capsular contracture or implant rupture. For this reason, Dr. Berman advises women under the age of 40 to avoid an axillary incision for their breast augmentation because, if and when the implants are removed and replaced, the underarm incision cannot be used again. Therefore, replacing the implants would result in two scars: under the arm and under the breast. While the scars do fade with time and Dr. Berman’s suturing technique creates as thin a scar as possible, if you’re at an age that replacement may happen in your future, placing the incision in the inframammary fold or around the areola may be a better choice for you. Who is a Candidate for Breast Augmentation?

Healthy adult women with realistic expectations are good candidates for breast augmentation. By that, we mean that one cannot expect to go from a 32A bra size to a 38DD bra size with one augmentation. It’s our goal at Berman Plastic Surgery and Spa that each breast augmentation patient’s results look both beautiful and natural as if her breasts were never augmented. If your goal is to have very large breasts and you have a very short and/or thin frame, it’s not impossible to “go large.” However, it may require more than one procedure. Aging, childbearing, breastfeeding, and genetics play their parts in terms of breast sagging. Some women have always had sagging breasts, while others experience this phenomenon after bearing and nursing children. There are specific measurements that Dr. Berman will perform in your cosmetic surgery consultation to identify whether or not your results would be improved by having a breast lift with your breast augmentation. It is never Dr. Berman’s goal to convince you to have an additional procedure if you be satisfied both with your experience and with your results. How is Breast Augmentation Performed?

All cosmetic surgery procedures are performed in the accredited ambulatory surgical facility in our Boca Raton plastic surgery suite. The American Academy of Ambulatory Surgical Facilities certifies our operating room. Dr. Berman has handpicked the most experienced surgical scrub techs and surgical and recovery room nurses to assist him, to assure the safest experience possible. He utilizes the services of board certified anesthesiologists, rather than opting for anesthesia nurses. Dr. Berman has over 30 years of experience, and our patients are as safe here as in a hospital operating room, while they avoid exposure to hospital-based bacteria. Additionally, they enjoy a much more personalized experience. This level of experience is hard to find with our combined 60 years’ experience in in-office ambulatory surgery. We have a 0-infection rate.

After marking the patient, general anesthesia is instituted by a board certified anesthesiologist. Incisions are then made in the area that you and Dr. Berman agreed upon during your consultation. A “pocket” is created, usually under the breast muscle, to house the breast implant, after which the implant is inserted. The breasts are closely and meticulously evaluated to be certain that both breasts look as identical as possible (almost every woman’s breasts are slightly asymmetrical), and when satisfied that the best result has been achieved, the incisions are closed with absorbable sutures and a dressing is applied. A special surgical bra is then put on the patient and she is transferred to the recovery area, where she will remain for a couple hours or until she is feeling ready to go home to recover.

Recovery and Results

Following breast augmentation, patients will be placed in a support bra or garment that will help to support their new breast contours. Breasts may feel tight and sensitive, and swelling and bruising may initially occur, which will dissipate over time. Pain medications can help with any discomfort that occurs. You should be able to walk without help after surgery, and it is important that you get up to walk every few hours to reduce the risk of blood clots. Many patients can return to work after a few days to a week, depending on the nature of the job. It is important that patients refrain from lifting, pulling, or pushing, and minimize excessive physical activity for the first couple of weeks to ensure optimal results. Following Dr. Berman’s post-op instructions is important for a smooth recovery period.
